Filing a Claim
In the event of an emergency, or any time that you are unable to contact AXA Assistance before seeking medical treatment, you may submit a claim for reimbursement after the fact. It is very important to keep all documentation that you receive from your provider, including receipts, medical records, etc.

There are two forms that you may need to submit as part of the process, depending on the circumstances: the medical claim form, for any expenses incurred for medical care, and the trip cancellation or interruption claim form, for certain covered travel expenses.
After you have filed your claim, you may check on its status by following the steps outlined in this document.
